Cheryl Taylor, Case Management Coordinator at Nutley Family Service Bureau (NFSB), can help clients and local residents make sure diverse needs are met through coordination of services, education, advocacy, and support. She’ll listen to your story and help you determine your eligibility for various social services and assistance programs.
She comes to NFSB from Robert Wood Johnson Barnabas Health, where she manages patient flow. Her primary responsibilities are to admit and discharge patients and develop discharge plans in collaboration with case management teams. Although many patients can go straight home from the hospital, many require additional care, whether through rehabilitation or long-term care. Cheryl helps them along on their journey.
Cheryl also served as a service coordinator at Tri-Borough Home Care, advocating for patients and ensuring compliance with regulations for seniors and people living with disabilities. She often dealt with the Traumatic Brain Injury Waiver Program and the Nursing Home Transition and Diversion Waiver Program, the latter of which is relevant to the senior population in Nutley.
“Many seniors worked their whole life and are now supposed to be in their golden years of retirement,” Cheryl said. “They have Medicare and may have secondary insurance, but they’re often hit with crazy medical bills or they can’t afford to pay for prescriptions. I can fill that gap by helping them navigate the system and figure out which state or local programs are good options for them.”
As Case Management Coordinator at NFSB, Cheryl believes in being as proactive as possible when people seek help. With a relatively high active caseload, Cheryl is working to prioritize client files and optimize the level and frequency of service to ensure the immediate needs of all clients are met. This could involve everything from financial assistance for food or rent, to legal services, to filing insurance claims.
Although some clients are dealing with very intense circumstances, Cheryl is motivated by the support of the NFSB team and the Nutley community.
“I love that NFSB is a smaller, family-oriented nonprofit,” Cheryl said. “Everyone here knows each other’s name. When I go for a walk, someone will honk their horn, and it’ll be the person who came into the office for help filling out an application. I love the community feel here.”
Cheryl always tries to go above and beyond, providing NFSB clients with multiple options to address their challenges whenever possible. She believes having more resources will help clients get better, faster results, which is especially important when facing pressing situations.
“Just having a conversation with people and sharing information makes them feel like they have more control over a situation,” Cheryl said. “Instead of handing someone an application or a phone number, I’ll help them fill out an application or get on the phone with them. I enjoy listening to their stories and helping them get the help they deserve.”
